A senior home operator has forfeited licenses at three facilities — in Pequot Lakes, Kimball and Adrian — after state inspectors found repeated incidents of serious harm to residents. Among them, a woman with Alzheimer’s was sexually assaulted by a male caregiver at the Heritage House facility in Pequot Lakes, and a man died at that same facility because of improper medical practices. The Star Tribune reports residents at Heritage House in Pequot Lakes, Kimball and Adrian will still be allowed to live there, but care will be provided by a different entity under closer state supervision.
